ConnectWell is a new Community Information Exchange® (CIE) serving East Central Ohio – Carroll, Columbiana, Holmes, Stark, Tuscarawas and Wayne counties.
ConnectWell CIE of East Central Ohio partners with local agencies, providers, and community members to link individuals with housing, food, transportation, healthcare, legal aid, and other essential resources. The initiative focuses on whole-person care — removing barriers, fostering collaboration, and creating a healthier, more connected community for everyone.
