The Homeless Continuum of Care

PartnerSolutions Homeless Navigation carries out two important functions in the Homeless Continuum of Care of Stark County (HCCSC):

1) The Homeless Navigation Hotline serves as Coordinated Entry (CE), operating to streamline and prioritize access to shelter and housing for people experiencing homelessness. The power of Coordinated Entry lies in cooperation between local agencies and service providers addressing homelessness and housing insecurity with a shared understanding of guiding principles. This allows separate organizations in the homeless continuum and the community to work together as one unit instead of making individual decisions about who to serve and how best to serve them. CE is an integrated process that streamlines access to resources in a community with the goal of equitable access to limited resources.

2) The HMIS Lead administers the Homeless Management Information System (HMIS), fulfilling 6 separate roles: Project Management, System Administration, Training, Helpdesk Support, Data Analysis and Reporting, and Communications.

The Homeless Continuum of Care of Stark County (HCCSC) is a coalition of individuals and organizations committed to ending homelessness in Stark County, Ohio. HCCSC is managed by Stark Housing Network Inc., an independent non-profit organization.
